Violeta Jardin: Measuring Antifibrinolytic Effect to Improve Tranexamic Acid Safety

“Tranexamic acid (TXA) is central to bleeding control in cardiac surgery, yet high cumulative dosing around cardiopulmonary bypass is linked to perioperative seizures and prolonged antifibrinolytic shutdown, particularly in patients with renal impairment.
This new case report of TXA-induced seizure immediately after CPB highlights the need for individualized TXA strategies and real-time assessment of its neuro- and haemostatic impact.
The ClotPro TPA-test offers the only viscoelastic quantification of TXA-induced inhibition of tPA-triggered fibrinolysis, allowing titration to effect rather than fixed dosing and supporting safer, lower-dose TXA regimens in high‑risk patients.
Yoshii et al. show that even a 1 g TXA bolus produces measurable, sustained antifibrinolytic activity, with TPA-test parameters closely tracking impaired eGFR—providing a practical tool to avoid TXA overdosing and balance bleeding, seizure, and thrombotic risk post‑cardiac surgery.”
