• The VTE-PREDICT score was designed to estimate recurrence and bleeding risks after acute VTE, but required validation in real-world populations.
• In 17 850 patients (median age 67 years; 64% with PE) from the RIETE registry, the model’s performance was tested for outcomes over 1–5 years under varied anticoagulant regimens.
• The score showed good discrimination for recurrence (c = 0.70–0.73) and moderate for bleeding (c = 0.65–0.67) but underestimated recurrence and overestimated bleeding in high-risk patients. Calibration refinement may enhance its clinical utility.”
