Michael Hadley: Well-Timed ACC Statement Just Out in JACC Journals
Michael Hadley, System Director of Advanced Cardiac Imaging at Northwell Health, shared on LinkedIn:
”Well-timed ACC Statement just out in JACC Journals on reporting Quantitative Coronary Plaque Analysis (QCPA)
Key takeaways:
- Use QCPA only when visual plaque is present to enhance risk assessment and guide initiation or intensification of preventive therapies
- Required in reports (see figure): TPV, NCPV, CPV, qualitative statement about overall plaque, list of excluded segments, and comparison to any prior
- To improve specificity, high-risk plaque is redefined as mild-moderate stenosis with both positive remodeling (>1.2x) and lipid-rich plaque (<70 HU)
- Serial QCPA could be considered after 2-5+ years if it would change management, with plaque progression defined as ≥10-20mm3 of TPV or ≥5-10mm3 of NCPV
- We need a shared validation dataset including IVUS/histology to evaluate the growing number or QCPA platforms
Bottom line: A strong first step toward standardized plaque reporting and building real trust in a tool built to personalize prevention.”
