”The first academic collaboration of 2026 began in Namma Chennai with a very well-attended CBC (Chennai Blood Club) meet today.
The meeting featured excellent interactive sessions, with the highlight being a challenging case of acquired haemophilia in an elderly patient on Hemlibra (emicizumab).
The discussion focused on how emicizumab interferes with routine clot based coagulation assays causing spurious shortening of APTT, falsely elevated Factor VIII levels, and unreliable Bethesda assays, making conventional monitoring of acquired hemophilia therapy almost impossible .
In patients on a Emicizumab, FVIII assays must be performed using bovine factors, two-step chromogenic methods.
Chromogenic FVIII assays provide accurate results in patients on emicizumab, whereas clot-based assays yield falsely normal values due to drug interference.
It gives me immense clinical satisfaction that we at the CRL Coagulation Laboratory were able to rapidly decode the puzzle, supporting both the clinician and, most importantly, the patient.
This case also strongly emphasized the critical importance of laboratories being aware of the patient’s clinical background and ongoing therapy before performing and interpreting coagulation tests.
Other stimulating cases discussed included:
* LAHPS syndrome (Lupus anticoagulant-hypoprothrombinemia syndrome )
* T-ALL: transplant decision-making using single-cell genomic data
* Renal amyloidosis spotter images
Overall, it was a rich clinical and academic feast, reinforcing the power of close clinician–laboratory collaboration.”
