Study Population: 198,473 participants
Follow-up: 5,248,916 person-years
Incident CHD cases: 20,033

Low-Carbohydrate Diet (Highest vs Lowest Quintile)

Healthy LCD (HLCD):

  • HR 0.85 (95% CI 0.82–0.89)
  • 15% lower CHD risk

Unhealthy LCD (ULCD):

  • HR 1.14 (95% CI 1.09–1.20)
  • 14% higher CHD risk

Animal-based LCD (ALCD):

  • HR 1.07 (95% CI 1.02–1.12)

Vegetable-based LCD (VLCD):

  • HR 0.94 (95% CI 0.90–0.99)

Low-Fat Diet (Highest vs Lowest Quintile)

Healthy LFD (HLFD):

  • HR 0.87 (95% CI 0.83–0.91)
  • 13% lower CHD risk

Unhealthy LFD (ULFD):

  • HR 1.12 (95% CI 1.07–1.17)
  • 12% higher CHD risk

Vegetable-based LFD (VLFD):

  • HR 0.87 (95% CI 0.83–0.91)

Metabolomic Correlates

Healthy LCD/LFD patterns were associated with:

  • Decreased Triglycerides
  • Increased HDL-C
  • Decreased hs-CRP
  • Increased 3-Indolepropionic acid
  • Decreased Valine

Unhealthy patterns showed the opposite metabolic signature.”
