Indunil Karunarathna: Kawasaki Disease at the Intersection of Immunology, Cardiology, and Vascular Biology

Indunil Karunarathna, Founder of Uva Clinical Research Lab (UCRL), shared on LinkedIn about a recent article he and his colleagues co-authored, adding:

Kawasaki disease is a unique pediatric inflammatory disorder that occupies a critical intersection between immunology, cardiology, and vascular biology.

Although classically described as an acute, self-limited febrile illness of childhood, it is now understood to be a complex immune-mediated vasculitis with the potential for lifelong cardiovascular consequences.

The disease process is driven by dysregulated innate and adaptive immune responses that target the vascular endothelium, particularly within the coronary arteries, leading to aneurysm formation, thrombosis, and long-term vascular remodeling.

Early diagnosis and timely immunomodulatory therapy, especially intravenous immunoglobulin, dramatically reduce morbidity and mortality, transforming outcomes from historically fatal to largely survivable.

However, survival does not equate to cure.

Even in patients whose coronary aneurysms regress, persistent endothelial dysfunction and vascular scarring confer long-term cardiovascular risk.

Kawasaki disease should therefore not be conceptualized as a transient pediatric illness, but rather as a lifelong cardiovascular condition originating in childhood, requiring structured acute management, long-term surveillance, and transitional care into adulthood.”
