”When do we actually use Dual Anti-Platelet Therapy (DAPT) in stroke prevention?
The guidelines have evolved significantly, moving beyond simple aspirin use for many patients. Here’s a concise breakdown of the current AHA/ASA recommendations — essential for any physician managing these cases.
- Minor Stroke / High-risk TIA: If a patient has a minor stroke (NIHSS ≤ 3) or a high-risk TIA (ABCD² ≥ 4), the evidence from the CHANCE and POINT trials supports starting DAPT within 24 hours. This aggressive approach continues for 21 days, which is the highest-risk period for recurrence.
- After Carotid Intervention (CAS): Following carotid artery stenting, DAPT is mandatory to prevent early stent thrombosis while the vessel heals. According to CREST protocols, this duration is typically 30 days.
- Intracranial Large-Vessel Stenosis: For symptomatic 70-99% stenosis of a major intracranial artery, the SAMMPRIS trial indicates that aggressive medical therapy is effective. Here, DAPT is recommended for a full 90 days.
The key takeaway? Timing and duration are paramount, always balancing the significant reduction in recurrent ischemic events against the increased bleeding risk.”
