Vanessa Simioni Faria: Precision Anticoagulation and Expanding Donor Frontiers

Vanessa Simioni Faria, Postdoctoral Research Fellow Brigham and Women’s Hospital, shared a post on LinkedIn:

”ISHLT2026 – Day 3 Insights

From anticoagulation strategies in LVAD patients to redefining donor eligibility, the field is moving toward precision, safety, and smarter decision-making across the entire patient journey.

  • SESSION 23 – Adverse Events Following Temporary and Durable Mechanical Circulatory Support in Adults

A few key highlights:

  • Anticoagulation in HM3 – DOT-HM3 (1-year results)

Presented by Zuzana Tucanova, MD; the study explored DOAC use (apixaban) in HM3 patients BTT:

  •  In selected, stable patients, apixaban appears safe; however, there were higher rates of Blood transfusion, temporary MCS, renal replacement therapy, and longer hospital stays

A promising alternative to VKA; but caution is clearly warranted.

  • Stroke and Preconditioning Strategies
  • In a study presented by Yuriy Hrytsyna, preconditioning with transaxillary microaxial flow pump  prior to durable LVAD implantation:
  • Was NOT associated with increased postoperative stroke risk. Similar outcomes compared to no preconditioning.

SYMPOSIUM 34 – Taking Donor Expansion, Procurement, and Preservation to the Next Level

One of the presentations focused on pushing the boundaries of the donor pool: (presented by Dr. Ann Woolley)

  •  HIVplus Donors
A thought-provoking discussion highlighted that the barrier is no longer biological; but social, regulatory, and perceptual.”
