“Atherosclerosis.
A heterogenous degenerative ,progressive disease of the arteries characterized by lipid deposition subintimally.
RFs.
Dyslipidaemia, Systemic hypertension, turbulent flow, Age, Gender at birth, Race, genetic predisposition, lifestyle and metabolic disturbances including diabetes mellitus, hyperhomocysteinemia.
Etiopathogenesis.
Classic theory:
Initiation
LDL cholesterol is deposited in the sub intima which becomes oxidized and ingested by macrophages (foam cells) which persistent accumulation leads to stenosis of arterial lumina with formation of plaque
Conplications
Chronic atherosclerosis tends to form a hypertrophic muscularis, alongside a fibrotic cap ,with tendencies of hemorrhage, thrombosis and arterial wall sclerosis.
When the thrombi detach,there is a risk of embolism and ischemic necroses to the corresponding tissue.
We observe atherosclerosis in most arterial networks:
aorta, iliac arteries, coronaries, carotids.
They produce a clinical picture accordingly:
eg. CAD, Cerebrovascular disease, mesenteric ischemic, claudicattio intermittens etc.
