“When celiac disease becomes a life-threatening emergency

‘Celiac Crisis Complicated by Refeeding Syndrome: A Case Report and Pediatric-Adapted Diagnostic Criteria’
In this study, we report the case of a 23-month-old child presenting with an exceptionally severe form of celiac disease, complicated by:
- Refeeding syndrome
- Deep vein thrombosis
- Intussusception
- Metabolic and nutritional derangement
Although celiac crisis is now considered rare in high-income countries, this case underscores how rapidly pediatric patients can deteriorate, and how challenging diagnosis and management can be in real-world clinical settings.
Key messages from our report:
- Early recognition of celiac crisis is critical
- Severe celiac disease may present even with initially low antibody titers
- Nutritional rehabilitation itself can be hazardous without close biochemical monitoring
- Complications may be multisystem and rapidly evolving, requiring high clinical vigilance
- Multidisciplinary management is essential in complex pediatric cases
We also propose pediatric-adapted diagnostic criteria to support earlier recognition and improve standardization in both clinical practice and future research.”
