“Low platelets… but the marrow is fighting back?
Think: Immature Platelet Fraction (IPF)
In hematology, the Immature Platelet Fraction (IPF) measures the percentage of newly released, RNA-rich platelets circulating in the blood.
These young platelets are the platelet equivalent of reticulocytes in anemia — giving us a real-time glimpse into marrow platelet production.
Why is IPF important?
Thrombocytopenia generally happens through two major mechanisms:
- Decreased platelet production: Bone marrow is not producing enough platelets.
- Increased platelet destruction or consumption: Platelets are being destroyed, consumed, or lost faster than normal.
This is where IPF becomes clinically useful:
High IPF suggests active marrow response and increased platelet production.
Often seen in:
- Immune thrombocytopenia (ITP)
- Peripheral platelet destruction
- Consumptive processes
- Recovery after marrow suppression or chemotherapy
Low or normal IPF may suggest impaired marrow production
Such as:
- Aplastic anemia
- Marrow infiltration
- Chemotherapy-related suppression
In practice, IPF helps answer an important clinical question:
- Is the marrow failing to produce platelets… or
- Is the body destroying and consuming them peripherally?
But IPF should never be interpreted alone.
A proper thrombocytopenia workup still begins with:
- CBC
- Peripheral smear review
- Excluding pseudothrombocytopenia from platelet clumping.”
