A preterm twin with congenital diaphragmatic hernia developed iliofemoral and renal artery thrombosis following umbilical arterial catheter use, leading to unilateral renal infarction.
Prompt recognition, catheter removal, and anticoagulation with heparin followed by enoxaparin preserved limb viability and right kidney function.
The case underscores the need for vigilance for thrombotic complications in neonates with central lines and complex postoperative courses.
