“A new finding published in Blood (April 2026) adds an unexpected dimension to the FXI story — directly relevant to one of ISTH 2026‘s hottest topics.

Researchers discovered that estrogen upregulates lactoferrin — an iron-binding glycoprotein traditionally associated with innate immunity — which in turn potentiates FXIa activity.

This mechanism may help explain why elevated estrogen levels correlate with elevated VTE risk in pregnancy, oral contraceptive use, and postmenopausal hormone therapy.

The significance: it reframes our understanding of hormonal regulation of coagulation and introduces FXI as a potential therapeutic target for thrombosis prevention in hormonal contexts — without necessarily impairing normal hemostasis.

This connects directly to the broader FXI inhibitor story dominating Phase 3 trials right now.
