What should we do when guideline-supported treatment fails, but the patient is still fighting for life?
Our newly published case report describes a patient with massive pulmonary embolism who remained critically ill despite standard thrombolytic therapy.
With no access to catheter-directed therapy or surgical embolectomy, repeat systemic thrombolysis became the rescue strategy; and the patient recovered.
One case cannot change clinical practice, but it can contribute to the evidence that shapes future decisions.
We hope this report adds to the discussion on managing massive pulmonary embolism, particularly in resource-limited settings.
