Mihaela Cordeanu: Can D-dimer Help Diagnose Splanchnic Vein Thrombosis?

Mihaela Cordeanu, Associate Professor and Hospital Practitioner in Vascular Medicine at Strasbourg University Hospital, shared a post on LinkedIn about a recent article she and her colleagues co-authored, published in JVS-Vascular Insights, adding:

“Can D-dimer help diagnose Splanchnic Vein Thrombosis?

Pleased to share our systematic review published in JVS-Vascular Insights, co-authored with Simon Soudet and Prof Dominique Stephan.

We analyzed 25 studies (9,052 patients) evaluating D-dimer diagnostic accuracy for SVT.

Key findings:

  • Highly variable performance (sensitivity 54–95%, specificity 33–96%)
  • Accuracy declines with worsening hepatic function, a clinical paradox, as advanced cirrhotic patients carry the highest PVT risk yet show the poorest D-dimer performance
  • High negative predictive value only in selected low-risk populations (Child-Pugh A)
  • Multimarker models (D-dimer and P-selectin or von Willebrand factor) outperform D-dimer alone but remain research tools

Bottom line: D-dimer cannot serve as a universal standalone rule-out test for SVT. Imaging remains the diagnostic cornerstone, consistent with major international guidelines.”
