In this review, the evidence for open ADAMTS13 as a highly specific biomarker for both acute and subclinical iTTP was summarized. Beyond its diagnostic value, we highlighted how ADAMTS13 conformation may contribute to early relapse risk prediction and help guide pre-emptive therapeutic interventions.
