Ahmed Kotb: A Practical Guide to Von Willebrand Disease

”Von Willebrand Disease (VWD)

Type 1: Partial decrease in von Willebrand factor (VWF) – usually responds to desmopressin (DDAVP).

Type 2: Qualitative defect in VWF.

  • Type 2A: Decreased high-molecular-weight (HMW) multimers.
  • Type 2B: Increased binding to platelet glycoprotein Ib (GPIb), resulting in thrombocytopenia; avoid DDAVP.
  • Type 2M: Decreased platelet adhesion.
  • Type 2N: Decreased binding to factor VIII (FVIII).

Type 3: Complete absence of VWF, causing severe disease; treat with VWF/FVIII concentrate.

Laboratory findings: Decreased VWF antigen, decreased VWF activity, and factor VIII may be decreased.”
