“Wondering when to start anticoagulation after dAVF embolization?
There are no universally accepted imaging criteria for starting anticoagulation after dAVF embolization.
The decision must be individualized based on venous drainage and thrombosis risk.
Strongly consider prophylactic anticoagulation for:
- High-grade dAVF with cortical venous drainage, such as Borden type II or III
- An abrupt change in venous outflow or marked venous stasis
- A large venous pouch or venous ectasia
- Venous sinus thrombosis or severe stenosis
Practical Takeaway: If embolization creates a low-flow, stagnant, thrombosis-prone remnant, anticoagulation may be considered. Always individualize based on angiographic findings, bleeding risk, and clinical status!”
