The physiology is the key: TXA blocks the fibrin-dependent tPA route of plasmin generation, so it’s most rational in hyperfibrinolytic hemorrhage.
In dilutional coagulopathy it does nothing — you need fibrinogen, plasma, platelets. In consumptive coagulopathy it can worsen microvascular thrombosis. And benefit disappears (harm becomes possible) beyond the 3-hour window from birth.
