What’s new in the 2026 guideline?

1. A more systematic approach to diagnosis

The guideline emphasizes comprehensive evaluation of anemia and identification of potentially reversible causes, particularly iron deficiency.

2. Updated approach to iron deficiency KDIGO introduces updated terminology:

  • Systemic iron deficiency
  • Iron-restricted erythropoiesis

This reflects a more precise understanding of iron availability and erythropoiesis in CKD.

3. Individualized iron therapy

The guideline provides updated recommendations for the use of oral and intravenous iron, with treatment decisions guided by iron status, clinical context, benefits, risks, and patient preferences.

4. Reassessment of anemia therapies The 2026 update incorporates newer evidence surrounding:

  • Erythropoiesis-stimulating agents (ESAs)
  • HIF-prolyl hydroxylase inhibitors (HIF-PHIs)
  • Iron therapies
  • Red blood cell transfusion strategies

5. ‘Treat the patient, not just the number’ A key theme is individualized anemia management—balancing symptoms, potential benefits, cardiovascular and treatment-related risks, comorbidities, and patient preferences rather than pursuing hemoglobin correction in isolation.”
