“KDIGO 2026 Anemia in CKD Guidelines

For the first time since 2012, KDIGO has released a comprehensive update to the Clinical Practice Guideline for Anemia Management in Chronic Kidney Disease!

The updated guideline incorporates over a decade of new evidence and introduces major practice-changing recommendations
key takeaways:

  • Hb Thresholds: Alignment with WHO standards—Anemia is defined as Hb < 13 g/dL in men, and < 12 g/dL in women.
  • Shared Decision-Making: A major shift away from fixed Hb thresholds for non-dialysis CKD. Treatment initiation is now highly individualized based on symptoms, cardiovascular risks, and the potential harms of ESAs or transfusions.
  • Proactive IV-Iron Regimens: Inspired by the PIVOTAL trial, the guideline now couples standard targets (Ferritin ≤ 500 ng/mL / TSAT ≤ 30%) with a proactive monthly 400 mg IV iron regimen (withheld if Ferritin > 700 ng/mL).
  • Expanding the Arsenal: HIF-PHIs (Hypoxia-inducible factor–prolyl hydroxylase inhibitors) are now officially integrated into the treatment algorithms alongside traditional ESAs.
