“Cerebral Venous Thrombosis (CVT): Think Early, Treat Decisively

Cerebral venous thrombosis is an uncommon but potentially devastating neurological emergency—and it can be easily missed because its presentation is often nonspecific.

Key clinical principles:

  •  Consider CVT in unexplained headache, seizures, focal neurological deficits, or raised intracranial pressure.
  •  MRI and MR venography (MRV) or CT venography (CTV) is essential for confirmation.
  •  Anticoagulation remains the cornerstone of acute treatment—even when venous intracranial hemorrhage is present.
  •  Identify and address underlying provoking factors and thrombophilia when appropriate.
  •  Recognize complications early: seizures, raised ICP, neurological deterioration, and venous infarction.
  •  In selected patients who deteriorate despite optimal anticoagulation, endovascular therapy may be considered in experienced centers.

Clinical pearl: A hemorrhagic venous infarction in a patient with suspected CVT is not, by itself, a reason to withhold anticoagulation.

Early recognition and appropriate anticoagulation, aggressive management of complications lead to better outcomes.”
