Amr Saad: What Tips You Toward Cerebral Venous Thrombosis Over IIH?
Amr Saad, Founder of PatientEd, Refractive Manager at Breyer, Kaymak and Klabe Eye Surgery, shared a post on LinkedIn about a recent article by Martins A.I. et al., published in Neurological Sciences, adding:
”Two patients present with papilledema and raised pressure. One turns out to have a cerebral venous sinus thrombosis, which is life-threatening.
The other has idiopathic intracranial hypertension, which is usually benign. Early on they can look the same.
A small study asked what actually separates them.
The surprising part is that the thrombosis patients had a lower papilledema grade and thinner nerve fiber layer than the benign group, so the more dangerous condition often showed less swelling on the disc.
Here is the catch. The single strongest separator was time to observation, which reflects how fast a thrombosis reaches care more than any sign on the optic nerve itself. With twelve thrombosis patients, this is a signal to keep in mind, not a rule to lean on.
It is still the kind of pattern I want in my head before the next swollen disc comes in.
