Marios Psychogios: Every Minute Matters in Acute Stroke Care
Marios Psychogios, Head of Diagnostic and Interventional Neuroradiology at Clinic of Radiology and Nuclear Medicine, University Hospital Basel, shared a post on LinkedIn:
“32 minutes. Imaging to reperfusion.
That’s the time from detecting the vessel occlusion to completing the thrombectomy — for one of our stroke patients in Basel.
To put that in perspective: the recent DISTAL trial reported a median imaging-to-reperfusion time of 115 minutes. We cut that by more than half — saving over an hour.
Why does this matter?
Every minute saved in this window meaningfully raises the odds of a better clinical outcome. Faster reperfusion has consistently been linked to smaller infarct size and improved functional recovery.
Ten years of conviction, now backed by evidence
We performed our first one-stop management case — imaging and thrombectomy in the same angio suite, no transfer between rooms — back in 2016. A decade later, I’m still convinced this is the right way to treat stroke patients.
But conviction alone doesn’t help patients. Evidence does.
Following the promising results of the SPINNERS trial on flat-detector CT for hemorrhage detection, we designed and launched GET-FAST — an international, multicenter, pragmatic randomized controlled trial comparing one-stop management to the standard two-room stroke workflow.
We just randomized our first patient in Basel.
Result: A patient who arrived severely affected (NIHSS 16) was completely asymptomatic (NIHSS 0) — within the hour.
