Ahmed Alotaibi։ Can the Storage Container Change Platelet Quality?

Ahmed Alotaibi, Senior specialist (Blood bank and blood transfusion) at Afif General Hospital – Riyadh Third Health Cluster (Ministry of Health), shared a post on LinkedIn  about a recent article by Janhavi Mahajan et al., published in Transfusion, adding:

”Can the storage container itself change platelet quality?

A 2026 study in Transfusion explored an important but often overlooked question in platelet research: whether cold-stored platelets behave differently depending on the container used for storage.

The investigators used 8 apheresis platelet components and divided each into three matched storage formats:

  • 15 mL polypropylene tubes
  • 30 mL mini-bags
  • Approximately 190 mL in the original platelet bag as the control

All components were stored at 2–6°C for 21 days and assessed for metabolic, structural, and functional quality.

The findings were striking.

Platelets stored in polypropylene tubes showed faster glucose consumption, greater lactate production, lower pH, mitochondrial dysfunction, increased activation, greater extracellular vesicle release, loss of important glycoproteins such as GPIbα and GPVI, and reduced clot strength during extended storage.

In contrast, the 30 mL mini-bags remained broadly comparable with the standard platelet bags, with no measurable functional deficit in clot formation over the 21-day storage period.

One finding I found particularly interesting was that the tube-stored platelets still maintained a pH above 6.4, despite clear deterioration in several other platelet quality parameters.

That raises an important QC question:

Is pH alone enough to assess platelet quality, especially for cold-stored platelets?

The study suggests that small volume itself may not be the problem. Container material, gas permeability, and storage geometry can significantly influence platelet metabolism and function.

This was a small in-vitro study involving only 8 donors, so it does not establish clinical equivalence or predict patient outcomes.

But the message is an important one:

The container is part of the storage environment, not just packaging.

Question for discussion:

When evaluating platelet quality, should we rely less on a single QC marker such as pH and place greater emphasis on functional and metabolic testing?
