“What is Nplate?

Nplate (romiplostim) injection is a prescription medicine used to prevent serious bleeding episodes in people with chronic immune thrombocytopenic purpura (ITP), an autoimmune disorder with an increased risk of bruising or bleeding.

Nplate is also used to treat patients who have been exposed to high levels of radiation (acute radiation syndrome) and have hematopoietic syndrome.

Nplate works by stimulating the body to make more platelets increasing platelet production.

The current HCPCS Level II code for Nplate (romiplostim) is J2802 (Injection, romiplostim, 1 microgram), effective January 1, 2025, which replaced the former code J2796. One billable unit equals 1 mcg.

Billing and Coding Details

  • HCPCS Code: J2802
  • Description: Injection, romiplostim, 1 mcg
  • Billing Units: 1 unit: 1 mcg. Total dosage administered must be calculated and reported in multiple units (e.g., a 250 mcg dose: 250 units).
  • Previous Code: J2796 (retired as of January 1, 2025)

Modifiers: Apply JW or JZ modifiers for discarded/wasted drug portions from single-use vials as mandated by Medicare guidelines.

Common Indications

Immune thrombocytopenia (ITP) in adult and pediatric patients with insufficient response to corticosteroids, immunoglobulins, or splenectomy.

Treatment of hematopoietic syndrome of acute radiation syndrome.”
