“Shelf-Stable Synthetic Blood: The Future of Emergency Transfusion?
A promising advancement in transfusion medicine is bringing freeze-dried, shelf-stable synthetic blood closer to human trials.
Kalocyte, in collaboration with the University of Maryland, Baltimore, is developing this technology with support from DARPA.
Why does it matter?
Conventional RBCs require 1–6°C storage and a continuous cold chain—limiting blood availability in:
- Battlefield and military medicine
- Prehospital trauma care
- Rural/remote emergencies
- Disaster and mass-casualty situations
- Air evacuation and austere environments
A new approach to artificial blood
Earlier hemoglobin-based oxygen carriers faced major safety concerns because free hemoglobin can cause vasoconstriction, inflammation and organ injury.
Kalocyte’s approach aims to create synthetic microscopic cells that mimic key RBC functions, including:
- Oxygen delivery
- Hemodynamic support
- Appropriate blood-flow properties
- Hemostatic function
- Minimal immune/thrombotic reactions
- Potential ABO-independent use
The game-changing feature
The product is being developed as a freeze-dried powder designed for prolonged room-temperature storage and rapid reconstitution when required.
Freeze-dried – store at room temperature – reconstitute – transfuse
This could bring blood-substitute technology directly to the point of injury rather than requiring a refrigerated blood supply.

- Remote healthcare
- Mass casualties
- Extreme environments
Current status
This remains an investigational technology. Human trials and rigorous evaluation of oxygen delivery, hemostasis, immunogenicity, thrombosis, organ safety and clinical outcomes are still essential.
The goal is not necessarily to replace donor blood, but to bridge the critical gap between hemorrhage and definitive medical care.
