Alexandru Stieber: Large-Vessel Vasculitis – Clinical Patterns and Treatment

Alexandru Stieber, Internal Medicine Resident at SUUMC Bucharest, shared a post on LinkedIn about a recent article by Patrice Cacoub et al., published in The Lancet, adding:

“A very useful review on large-vessel vasculitis (LVV) from The Lancet Group.

From detailing the clinical phenotypes and summarizing the main therapeutic approaches, this review also proves insightful for including ‘isolated aortitis’ as a clinically relevant entity.

  • Giant cell arteritis (GCA) – older adults, symmetrical, mostly thoracic aortic involvement (axillary predominantly), polymyalgia rheumatica as the most common association, complicates mostly by anuerysms.
  • Takayasu arteritis (TAK) – younger adults, asymmetrical involvement, predominant abdominal vascular involvement, tendency towards stenoses.
  • Isolated aortitis – ascending aorta predominantly, seldom constitutional.

Also, the article highlights important secondary etiologies of LVV and/or mimickers, such as infections, iatrogenic scenarios and systemic diseases such as the periaortitis from erdheimchester disease.”
