“Today I am sharing Chapter V — the penultimate installment of Blood Donation: Beyond Good Intentions.

Patients: Absent Even from Our Language

We talk about donors. Altruism. Voluntariness. Safety. Collection rates. Plasma. Regulations. Models. Targets.

But perhaps we have spent so much time discussing how blood should be collected that we have forgotten to ask the most important question:

Did patients actually receive what they needed?

A country could hypothetically achieve 100% voluntary non-remunerated blood donation and still have insufficient blood or plasma.

The metric could be perfect.

And the mission could still be failing.

That paradox lies at the heart of this chapter.

Perhaps the next evolution of our blood systems is not to abandon donor-centered metrics, but to finally subordinate them to what should always have been the ultimate measure of success:

  • patient-centered outcomes.

Because after all our principles, models, indicators and debates, the patient ultimately asks a much simpler question:

  • ‘When I needed it, was it there?’
