Omega-3 is not an exposure.

A recent paper associated omega-3 supplementation with accelerated cognitive decline in older adults.

A published letter from Simon Dyall and Mélanie Plourde details the missing data, residual confounding, implausibly discordant cognitive measures, absence of corresponding progression in Alzheimer’s biomarkers, and an exposure definition based on self-reported use at a single time point – with no adequate account of dose, duration, adherence, or fatty-acid composition.

This is not an isolated methodological problem. It is a recurring failure of biological resolution.

We saw it when an atrial-fibrillation signal associated primarily with high-dose pharmaceutical omega-3 in people with high baseline cardiovascular risk was translated into ‘omega-3 is bad for the heart.’

An endpoint-specific, dose- and substrate-dependent risk was generalized to cardiovascular disease, ASCVD, dietary fish, and ordinary nutritional supplementation.

But these are not interchangeable exposures. Dietary fish is not a capsule. A nutritional dose is not 4 g/day of a pharmaceutical preparation. EPA is not DHA. Reported supplement use is not an omega-3 index. A person with structural heart disease, bradycardia-dependent AF vulnerability, or concurrent beta-blockade is not the general population.

Even the newest 35-trial AF meta-analysis makes the distinction visible: the statistically significant signal appeared in high-risk patients receiving high doses, with an absolute increase under 1%. The other dose or risk strata were null, while several high-dose trials still produced cardiovascular benefits that equaled or exceeded the AF risk.

The question is not: ‘Are omega-3s beneficial or harmful?’

It is: which fatty acid, in what formulation and dose, producing what biological exposure, in which phenotype, against which endpoint, with what absolute risk and competing benefit?

When the exposure category is biologically incoherent, the headline will be too.”
