Wolfgang Miesbach: Haemophilia and Ageing – A Joint ISTH/EAHAD Session at ISTH 2026

“Haemophilia and ageing – a joint ISTH/EAHAD session at ISTH 2026.
As our patients grow older, the questions we face are no longer only about bleeding.
Three complementary perspectives – Thomas Caprio, Robert Klamroth and Andreas Strauss – mapped how ageing is reshaping haemophilia care.
Robert Klamroth’s talk on balancing the risks between thrombosis and bleeding captured the shift particularly well:
- The population has aged
In US registries, the more than 65 group has grown from nearly 2% (2011) to 5% (2023), while the less than 45s have fallen from 86% to 76%.
Cardiovascular disease is now a routine comorbidity, not an exception.
- Causes of death are changing
EUHASS 2024 data show cardiac events and intracranial haemorrhage alongside bleeding as leading causes – raising the key question: can we reduce these?
- Hypertension is more prevalent
Higher in PWH aged 50–59 vs the general population (H3 vs DEGS1), yet management is no different – and measurement is simple.
- ‘Naturally anticoagulated’ needs revisiting
In the prophylaxis and non-factor era, this assumption no longer holds.
Data (van der Valk, Blood Adv 2022; COCHE) suggest antithrombotic treatment can be delivered safely – with appropriate factor cover.
The open questions that will define the coming years:
- Are PWH still ‘naturally anticoagulated’ in the modern era?
- Do non-factor therapies alter thrombotic risk?
- Are rebalancing agents changing risk stratification altogether?”
