Abid Husain: Tirzepatide Drives Broad Improvements in Cardiovascular Risk Biomarkers

Abid Husain, Consulting Physician and Scientific Advisor at Hone Health, Medical Doctor at Boulder Longevity Institute, shared a post on LinkedIn about a recent article by Naveed Sattar et al, published in JACC Journals, adding:

“Fantastic new data with supporting tirzepitider in the SURMOUNT-1 Post Hoc analysis!

Tirzepatide now leads current incretin therapy in the magnitude of weight and waist reduction. SURMOUNT-5 established superiority over semaglutide.

This new SURMOUNT-1 analysis adds a broad, quantified remodeling of the pathways through which obesity becomes cardiovascular disease.

At 15 mg, tirzepatide produced placebo-adjusted reductions of 54.6 percent in hsCRP, 30.2 percent in IL-6, 39.1 percent in HOMA-IR, 61.4 percent in leptin, 32.7 percent in GGT and 39.9 percent in FGF-21, while adiponectin increased 47.7 percent.

Semaglutide reduced CRP by 39% to 48% across STEP 1–3. Cross-trial comparisons cannot establish biomarker superiority, but tirzepatide now provides a substantially broader map of reduced adipose dysfunction, hepatic stress, insulin resistance and inflammation.

The vascular findings strengthen the argument. E-selectin fell 26.4 percent, sICAM-1 11.1 percent and PAI-1 antigen 44.3 percent relative to placebo at 15 mg.

This pattern is consistent with less endothelial activation, leukocyte adhesion and fibrinolytic resistance.

These pathways connect visceral adipose dysfunction to atherosclerosis, thrombosis, microvascular disease and HFpEF.

Fibrinogen, tPA antigen and thrombomodulin remained unchanged, showing a selective biologic response across the cardiovascular system.

Most changes correlated with reductions in weight and waist circumference, although the relationships were generally moderate outside of leptin.

Weight loss explains part of the effect; it does not fully characterize the biologic response.

The SELECT trial showed improved biologic effects prior to weight loss.

Semaglutide retains the strongest hard cardiovascular outcomes evidence through SELECT.

Hard outcomes data are pending with upcoming Tirzepitide trials but this is as close as one may need to say that Tirzepitide is at least equal and may be better as a cardiometabolic therapy than Semaglutide.”
