“Vitamin B12 deficiency can present far beyond fatigue and macrocytic anemia.
Neurological changes such as paresthesia, impaired vibration or position sense and gait disturbance can develop before obvious hematologic abnormalities appear.
That makes clinical context especially important. Medication history, gastrointestinal disease, dietary intake and previous gastric or intestinal surgery can all influence B12 status.
When serum B12 results are borderline, methylmalonic acid can provide additional diagnostic value because it rises when intracellular B12 availability becomes inadequate.
Early recognition is particularly important when neurological symptoms are present, since prolonged deficiency can lead to persistent nerve dysfunction.
A useful reminder that interpreting micronutrient status requires more than reviewing one laboratory value.”

Vitamin B12 Deficiency: More Than Just Anemia
Vitamin B12 deficiency is a common but often overlooked condition that can affect both the blood and nervous system.
Common causes: Pernicious anemia, vegan diet, malabsorption, gastric/ileal disease, and certain medications such as long-term metformin.
Key signs:
- Fatigue and weakness
- Pallor
- Macrocytic megaloblastic anemia
- Glossitis
- Tingling or numbness
- Balance and gait problems
Diagnosis: CBC may show an increased MCV, while further testing can help identify the underlying cause.
Management: Vitamin B12 replacement – oral or intramuscular depending on severity and absorption – along with treatment of the underlying cause.
Early recognition is important, especially because neurological symptoms may develop alongside or even before severe anemia.”
