‘Subscapular Hepatic Hematoma and Extensive Venous Thrombosis Associated with Suspected Fascioliasis: A Case Report from Ethiopia’
This case presented a rare and life-threatening combination — a massive hepatic subcapsular hematoma, portal vein thrombosis, and extensive DVT — all in a patient where repeated stool examinations were negative for Fasciola hepatica.
The diagnosis was made on clinical grounds:
- Persistent eosinophilia peaking at 32.7 percent
- Occupational exposure to sheep
- History of consuming inadequately cleaned raw vegetables
Empiric triclabendazole combined with anticoagulation led to complete resolution at 6-month follow-up imaging — a reminder that clinical judgment remains our most powerful diagnostic tool, especially in resource-limited settings.
Key teaching points:
- Fascioliasis can cause rare but serious vascular complications
- Negative parasitology does NOT rule out the diagnosis
- Clinical history and eosinophilia are critical diagnostic clues
- Empiric treatment can be life-saving
