This quick reference chart highlights commonly used options including Apixaban, Rivaroxaban, Edoxaban, Dabigatran, LMWH, and UFH, with important renal function considerations.

Key clinical points:

  • Use Cockcroft Gault CrCl when making anticoagulant dosing decisions.
  • Reassess renal function regularly.
