Piotr Czempik: ROTEM Guided Anticoagulation in Severe Thrombocytopenia and Acute VTE

“I am pleased to share our newly published clinical image article in Polish Archives of Internal Medicine: Thromboelastometry guided anticoagulation in severely thrombocytopenic patients with venous thromboembolism: three illustrative cases.
Managing venous thromboembolism (VTE) in patients with severe thrombocytopenia remains a major clinical challenge.
Guidelines often recommend dose reduction or temporary interruption of anticoagulation, and platelet transfusion practices vary widely.
In this context, rotational thromboelastometry (ROTEM) can provide valuable real‑time insight into clot quality that conventional laboratory tests cannot fully capture.
In our article, we present three illustrative cases of patients with acute VTE and platelet counts below 50 billion cells per liter, in whom ROTEM helped guide individualized anticoagulation decisions.
Despite profound thrombocytopenia, ROTEM consistently demonstrated preserved platelet contribution to clot strength, allowing safe optimization of LMWH dosing and avoidance of unnecessary platelet transfusions.
This work highlights the potential role of viscoelastic testing in refining anticoagulation strategies in complex hematologic and oncologic patients.
