Hany Ibrahim: Lp(a) Lowering Meets a Critical Test in the Lp(a)HORIZON Trial

“Today’s announcement that the topline results from the Phase 3 Lp(a)HORIZON trial showed that it did not meet its primary cardiovascular endpoint is undoubtedly disappointing, particularly given the substantial unmet need among people with elevated lipoprotein(a).
My initial thoughts are:
- Even when extensive genetic and epidemiologic evidence supports a risk factor as causal, lowering it pharmacologically does not automatically guarantee improved clinical outcomes. The magnitude, timing, duration, and mechanism of reduction, as well as the population treated, can all matter.
- This was one study evaluating a specific therapy. The findings cannot yet be assumed to apply uniformly to every Lp(a)-lowering agent or therapeutic modality, although they will undoubtedly have important implications for the entire field.
- The full results will be essential. We need to understand more.
- This result raises the scientific and evidentiary bar for ongoing Lp(a)-lowering trials. Demonstrating profound biomarker reduction will not be enough; a meaningful reduction in cardiovascular events remains the ultimate measure of success.
- This outcome should not diminish the importance of recognizing elevated Lp(a) as a genetically determined and independent cardiovascular risk factor.
This is a sobering result, but not necessarily the end of the Lp(a) story.
It is a reminder that cardiovascular science advances by rigorously testing compelling biological hypotheses against outcomes that truly matter to patients.
