Marco Moscarelli: 3D Reconstruction Enhances Volumetric Assessment of Post-TAVI Thrombosis

Marco Moscarelli, Editor and Statistical Reviewer for European Journal of Cardio-Thoracic Surgery (EJCTS), shared a post on LinkedIn:

“This patient refused warfarin. Still active, totally asymptomatic. Yet 3D reconstruction of valvular and perivalvular thrombosis after TAVI may provide several important advantages.

  • First, it allows a true volumetric assessment of thrombus burden, expressed in mm³ and voxels.
  • Second, it provides information on CT attenuation in HU, which may give some indication of thrombus composition and evolution over time.

In this case, HALT volume increased from 717 mm³ to 1,129 mm³, while mean attenuation increased from 107 HU to 116 HU, suggesting a possible trend toward thrombus organisation rather than simply fresh apposition.

This niche can be difficult to appreciate on conventional 2D CT images, as shown in this video. Neo-sinus thrombosis also worsened over time.

Perhaps we should welcome the recently published ACASA-TAVI trial in JAMA and start thinking more about a precision approach to anticoagulation after TAVI.”
