Nathan Hutchinson-Jones: Pharmacist-Led Care Optimises Cancer-Associated Thrombosis

Nathan Hutchinson-Jones, Deputy Chair – Trust Thrombosis and Haemostasis Group, shared a post on LinkedIn:

“A great day at the VTE Together conference in Dublin today, where I had the opportunity to talk about:

‘Beyond the prescription: What pharmacists really do in cancer-associated thrombosis (CAT)’

As CAT management has evolved, we now have more anticoagulant options and greater patient choice, but this has also made clinical decision making more complex.

My talk explored how pharmacists can add value beyond prescribing: navigating drug interactions and bleeding risk, supporting procedures, managing complex patients, providing education and safety netting, and helping deliver sustainable models of shared care.

I also shared our experience of developing a dedicated multidisciplinary CAT service at the RUH, where over one in four documented reviews resulted in optimisation of anticoagulant therapy.

None of this happens in isolation. Our service relies on close multidisciplinary working, with our specialist nursing team playing a central role in delivering the clinic, alongside pharmacy support.

For me, the future of CAT care is pharmacist enabled, multidisciplinary and patient centred.

I also think there is an opportunity for nationally endorsed, comprehensive guidance for CAT management, alongside wider adoption of dedicated multidisciplinary CAT clinics as the standard model of care.

Ultimately, it’s about the right anticoagulant, for the right patient, at the right time — with ongoing review throughout the cancer journey.
