Nisheeth Patel: The Clinical Balance Between Clot Prevention and Bleeding

Nisheeth Patel, Senior Consultant Neurologist at Samvedan Neurology Centre, shared a post on LinkedIn:

“Antiplatelet or anticoagulant.

In stroke, it is never a reflex.

Every day in stroke practice, we walk a narrow road.

On one side is the clot.

On the other is the bleed.

I tried to put that tension into a short poem and turned it into a 60-second video:

We walk the narrow road, between clot and bleed.

A platelet holds a spark, thrombin weaves a net.

One drug quiets the platelet; another interrupts the clot.

But neither is the answer until the mechanism speaks.

Find the source, choose the path, protect the brain, without forgetting the bleed.

The message is simple.

The choice is driven by mechanism: cardioembolic, large artery, or small vessel.

And every decision to protect the brain from the next clot has to weigh the risk of the next bleed.

This was also an experiment in making medical education content with AI.

The music came from Suno, the artwork from ChatGPT, and the animation from Claude.

The clinical thinking, I hope, is still mine.

A question for those who teach this: For a patient with atrial fibrillation and a prior small bleed, what tips your decision, and how do you explain it to your residents?”
