” ‘5 CBC mistakes residents make’ series
CBC Mistake 3: Isolated thrombocytopenia is equal to ITP
ITP is a diagnosis of exclusion.
Before labeling it ITP, ask:
- Is the thrombocytopenia real? then check for platelet clumping
- Is it truly isolated? then review Hb/WBC and smear
- Any drugs, infection, liver disease or splenomegaly?
- Any schistocytes? then think microangiopathy
- Any blasts? then think marrow disease
‘Low platelets’ is a finding. ITP is a diagnosis.
The blood film can completely change the differential.”
