“For years we’ve celebrated red blood cell recovery. Perhaps it’s time to ask a more important question: where did the platelets go?
In cardiac surgery, hemoglobin is far more than a laboratory value. It is the engine of oxygen delivery (DO₂), sustaining tissue metabolism throughout cardiopulmonary bypass.
CPB itself places the patient’s blood under extraordinary stress. Hemodilution lowers oxygen-carrying capacity, hypothermia alters coagulation, hemolysis releases free hemoglobin, acidosis impairs enzymatic clotting, while inflammation and mechanical shear activate and consume platelets.
Conventional cell salvage systems were engineered to recover washed red blood cells, not platelets.
As a result, the vast majority of platelets are removed during the washing process, leaving a critical gap in hemostatic preservation.
