Consuela A. Albright: Building a More Connected Sickle Cell Care System

”Today’s spotlight is personal — this is my team.

Sometimes the most effective advocacy is redesigning the system itself — and Cleveland Clinic’s Sickle Cell Disease Medical Neighborhood is a local example of exactly that, one I’m proud to be part of.

The program was built around a simple but overdue idea: sickle cell warriors have historically navigated fragmented, reactive care — bouncing between the ER, inpatient stays, and outpatient visits with little coordination between them.

The Medical Neighborhood model replaces that with a dedicated team that follows patients throughout their lives, bridging emergency care, hospitalizations, outpatient visits, and specialty services into one coordinated experience.

I want to name the people who make that coordination real.

Medical Directors Ravi Talati and Crawford Strunk, who lead the clinical vision for this program.

RN Coordinator Katrina Seedhouse, who holds the day-to-day threads of patient care together.

Social Workers Rosa Young and Milvon Bryson who address the parts of this disease that a clinical visit alone never touches.

And CCF Pediatric Hematology Department Chair Rabi Hanna, whose leadership shapes how this care gets built from the ground up.

I’ve worked in outpatient oncology settings with sickle cell patients, and I’ve seen firsthand what happens when coordination doesn’t exist — the same history repeated to five different providers, gaps in follow-up, patients who stop trusting a system that keeps failing to talk to itself.

This team is advocacy translated into clinical infrastructure, right here in our own city, and I’m grateful to work alongside them.”
