Giacomo Buso: Silent Atherosclerosis Begins Earlier Than Expected

“Exciting data from REACT, highlighting that silent atherosclerosis is already identifiable in young adults and raising important questions about the potential role of peripheral vascular imaging in early disease detection.”

“How early does atherosclerosis begin, and where should we look for it?

The REACT study, presented as a Hot Line at ESC Congress 2026, provides striking data on silent atherosclerosis across adult life.

Among 16,808 adults aged 18 to 70 years without known ASCVD, silent atherosclerosis was detected in 57.1 percent overall and was already present in 8.7 percent of men and 6.7 percent of women aged 18 to 29 years.

Interestingly, peripheral vascular imaging may help identify atherosclerosis at an early stage.

In younger adults, atherosclerosis was most often detected in peripheral arteries and confined to a single vascular territory, while isolated coronary atherosclerosis was uncommon across age groups.

REACT does not yet tell us whether imaging based screening will improve clinical outcomes, but it certainly challenges us to think about when, and where, atherosclerosis begins.”
