Jeffrey Schnur: Advanced Treatment Strategies for Deep Vein Thrombosis and Pulmonary Embolism

Jeffrey Schnur, Cardiology Nurse Practitioner at University of North Florida, shared a post on LinkedIn:

“One thing I’ve become increasingly passionate about in cardiology is ensuring patients with significant DVT and pulmonary embolism have access to all appropriate treatment options.

Many hospitals do not offer catheter-based DVT and PE interventions.

Anticoagulation is often an appropriate treatment, but for some patients, the conversation shouldn’t stop there.

Large clot burden, significant symptoms, extensive iliofemoral DVT, right ventricular strain, and hemodynamic concerns may warrant evaluation by a team experienced in advanced PE and DVT therapies.

What concerns me is when patients are never given that opportunity because transfer or consultation is never considered.

The goal isn’t to perform thrombectomy on everyone.

The goal is to make sure the patient has access to a complete evaluation and understands all available options.

For some patients, untreated clot burden can result in long-term consequences including post-thrombotic syndrome, chronic venous symptoms, persistent exercise intolerance, chronic thromboembolic disease, pulmonary hypertension, and eventually right heart dysfunction.

Sometimes doing what’s best for the patient means picking up the phone and involving a center with additional expertise, even when continuing anticoagulation locally may be the easier path.

Patients deserve a discussion about what is possible, not just what is available at the facility where they happen to present.”
