Mohamed Magdy: Blood Transfusion Reactions – High-Yield Summary
Mohamed Magdy, Physician at Ministry of Health Oman, shared a post on LinkedIn:
“Blood Transfusion Reactions – High-Yield Summary
Acute Reactions (during or within 24 hours)
1. Acute Hemolytic Reaction (ABO mismatch)
- Fever, back pain, hypotension, hemoglobinuria, DIC.
- Positive DAT, increased LDH, decreased haptoglobin.
2. Febrile Non-Hemolytic (FNHTR)
- Fever and chills without hemolysis.
- Prevention: Leukoreduced blood.
3. Allergic Reaction
- Urticaria, itching, flushing.
- Treatment: Antihistamines if mild.
4. Anaphylaxis
- IgA deficiency, bronchospasm, hypotension, shock.
- Treatment: IM epinephrine.
5. TRALI
- Acute hypoxemia within 6 hours.
- Bilateral infiltrates, non-cardiogenic pulmonary edema.
- Treatment: Respiratory support.
6. TACO
- Fluid overload, hypertension, raised JVP, pulmonary edema.
- BNP often elevated.
- Treatment: Oxygen and diuretics.
7. Septic Reaction
- High fever, rigors, hypotension, shock.
- Obtain cultures and IV antibiotics.
Delayed Reactions (days to weeks)
- DHTR: Falling Hb, jaundice, positive DAT.
- TA-GVHD: Fever, rash, diarrhea, hepatitis, pancytopenia.
- PTP: Severe thrombocytopenia 5–10 days after transfusion – IVIG.
- Iron overload: Repeated transfusions – cardiac and liver damage; iron chelation.
- Alloimmunization: Antibodies against RBC antigens – future transfusion complications.
TRALI vs TACO – Exam Favorite
- Feature| TRALI| TACO
- Cause| Lung injury| Fluid overload
- BP| Low/normal| High
- JVP| Normal| Raised
- BNP| Usually normal| Elevated
- Diuretics| No routine benefit| Beneficial
Most Important: First Action
- STOP transfusion immediately.
- Maintain IV access with normal saline using new tubing.
- Assess ABCs and vital signs.
- Notify physician and blood bank.
- Send appropriate samples for investigation.
Memory Pearls
- Back pain and dark urine – Acute hemolysis.
- Fever only – FNHTR.
- Itching plus urticaria – Allergy.
- Hypoxemia plus bilateral infiltrates – TRALI.
- Hypertension plus raised JVP – TACO.
- Rash plus diarrhea plus pancytopenia – TA-GVHD.
- Delayed thrombocytopenia – PTP.”
