Parth Parmar: Practical Guidelines for Managing Venous Thromboembolism

“Venous Thromboembolism Guidelines Series | A Practical Clinical Apprroach
Venous thromboembolism (VTE) — encompassing deep vein thrombosis (DVT) and pulmonary embolism (PE) — requires structured diagnostic reasoning, timely treatment and careful assessment of recurrence and bleeding risks.
The Internal Medness VTE Guidelines Series will translate key guideline recommendations into practical clinical learning.
Topics covered:
- Diagnosis: Clinical probability scores, D-dimer and appropriate imaging
- DVT management: Diagnostic pathways and anticoagulation
- Pulmonary embolism: Haemodynamic assessment and risk stratification
- Anticoagulant selection: DOACs, LMWH, unfractionated heparin and vitamin K antagonists
- Duration of treatment: Provoked versus unprovoked VTE and recurrence risk
- High-risk PE: Reperfusion strategies and escalation of care
- Special populations: Cancer-associated thrombosis, pregnancy and renal impairment
- Prevention: Hospital-associated VTE, recurrence prevention and selected indications for thrombophilia testing
Key clinical principle:
Anticoagulation decisions should account for clot location and severity, haemodynamic status, bleeding risk, comorbidities and patient preferences. Treatment duration should be individualised rather than determined by a one-size-fits-all rule.
The series will draw on established guidance, including NICE and American Society of Hematology recommendations, while highlighting practical decision-making points.
Follow Internal Medness for concise, clinically relevant internal medicine education.
