Raghavendra Rao: Varicose Veins and Easy Bruising – Is There a Link?

“Easy bruising in patients with varicose veins (Insights from iSearch Biomed).
Easy bruising can occur with varicose veins, particularly as spontaneous bruising on the lower legs, but it is not specific to venous disease and should not automatically be attributed to varicose veins.
- In an observational study of 494 people with varicose veins or chronic venous insufficiency, spontaneous lower-leg ecchymosis occurred in 5%. It was associated with muscle cramps and was more frequent in women and younger participants. The authors proposed ecchymosis as a possible sign of venous disease, but the study was observational and cannot prove that varicose veins caused the bruising.
- A subgroup analysis of 600 participants from the VEIN-TURKEY study found ecchymosis in 16.6% of those with peripheral varicose veins versus 2.7% without varicose veins. The association remained significant after regression analysis, and the authors specifically considered bruising relevant when trauma, blood disorders or antiplatelet treatment, and arterial obstruction were absent.
Other explanations are important.
Easy bruising and varicose veins may occur together in connective-tissue disorders such as vascular Ehlers–Danlos syndrome, but the evidence here is mainly case reports or small clinical descriptions rather than studies of typical patients with varicose veins.
Easy bruising and varicose veins were also both reported in women with a lipedema phenotype; however, that cross-sectional survey cannot establish causation, and its participants were recruited specifically because of suspected lipedema.
The supplied evidence does not establish a reliable way to distinguish ordinary venous-related bruising from a bleeding disorder.
Medication-related bruising is another possibility, including with anticoagulants and some cancer treatments.
New, widespread, rapidly increasing, unusually large, or unexplained bruises therefore warrant clinical assessment rather than being assumed to result from varicose veins.
