Day 160/365 – Medicine of the Day: Streptokinase

Streptokinase is a thrombolytic (fibrinolytic) agent that helps dissolve dangerous blood clots by activating plasminogen and converting it into plasmin, which breaks down fibrin clots.

It has been widely used in acute thromboembolic emergencies to restore blood flow and reduce tissue damage.

Drug Class: Thrombolytic (Fibrinolytic Agent), Plasminogen Activator
Uses: Acute STEMI, Massive Pulmonary Embolism (PE), Deep Vein Thrombosis (DVT), Arterial Thromboembolism
MOA: Forms a complex with plasminogen → Converts plasminogen to plasmin → Degrades fibrin clots → Restores blood flow
Dose: STEMI: 1.5 million IU IV infusion over 60 minutes (dose varies by indication)
Adverse Effects: Bleeding, Hypotension, Allergic Reactions, Fever, Rare Hemorrhagic Stroke
Contraindications: Active internal bleeding, Recent hemorrhagic stroke, Major recent surgery or trauma
Drug Interactions: Anticoagulants, Antiplatelets, Other thrombolytics, NSAIDs (↑ bleeding risk)
Brand Names: Streptase, Kabikinase, Thromboflux

Clinical Pearl:

Streptokinase rapidly dissolves thrombi by promoting plasmin formation.

Early administration after symptom onset significantly improves outcomes in acute myocardial infarction, but careful monitoring is essential due to the risk of serious bleeding.”
