Safaa Hilal: Improving Postpartum Hemorrhage Outcomes Through Early Warning Scores

“MEOWS: The Score That Saves Mothers.
In postpartum hemorrhage (PPH), maternal deterioration is often subtle before it becomes catastrophic.
By the time hypotension is obvious, significant blood loss and tissue hypoperfusion may have already occurred.
The Modified Early Obstetric Warning Score (MEOWS) provides a simple, standardized bedside tool to identify clinical deterioration early—before irreversible organ dysfunction develops.
MEOWS continuously monitors key physiological parameters, including:
- Heart rate
- Blood pressure
- Respiratory rate
- Oxygen saturation
- Temperature
- Level of consciousness
- Urine output
- Additional obstetric concerns
Each abnormal parameter contributes to the total score, allowing clinicians to recognize trends rather than waiting for a single catastrophic event.
Why is MEOWS important in PPH?
- Detects occult shock early
- Triggers timely senior review and multidisciplinary response
- Standardizes communication using SBAR
- Promotes activation of the PPH bundle within the Golden Hour
- May prevent progression to endotheliopathy, coagulopathy, cytokine storm, and multiple organ failure
